Nottingham Forest fans have the chance to become a matchday guest of honour by supporting Prostate Cancer UK.
Representatives of the charity will attend Forest’s clash with Middlesbrough on Saturday 7 March to raise awareness and money for a disease which affects one in eight men.
And ahead of the match, Prostate Cancer UK would like to learn how much Forest fans know about prostate cancer and hear people’s experiences of the disease.
Fans who complete this short survey
will be entered into a draw to be the club’s guest of honour for the Middlesbrough match.
The deadline for entries is Thursday 5 March, after which one lucky participant will be drawn at random.
